This may seem really random, but have you considered food allergies or Celiac’s? This sounds exactly like my stepsister’s symptoms.

parasites or celiac (gluten intolerance) if there’s an underlining problem it won’t matter what or how much she eats if she’s not absorbing her food. stomach pains can be a lot fo things but not gaining weight and that scream gluten intolerance to me!!!
These are all questions I would ask, too. If she eats a lot and is incredibly thin, an inability to utilize nutrition is almost certainly a factor. Does she have digestive problems? If so, I would also consider the possibility of IBS or Crohn’s. Also research PCOS in addition to gluten intolerance and parasites.
